小児での造血の柔軟性
子どもの骨髄異形成症候群には、生殖細胞系列に含まれる素因が大きな役割を果たしている。今回S Sahooたちが、小児の骨髄異形成症候群の8％はSAMD9遺伝子とSAMD9L遺伝子の変異が原因であることを明らかにした。これらの遺伝子から生じる表現型パターンは重なり合う部分が多く、臨床転帰もほぼ同じである。原因となる変異は細胞の増殖を抑制し、somatic genetic rescueと呼ばれるまれな遺伝的救済を伴い、それが患者の61％に見られる不均一なパターンのクローン性造血に関連している。このような高率の体細胞モザイク現象は、ヒトの疾患では前例がない。表紙は、モザイク現象の概念と、小児の造血の並外れた可塑性を表現している。
